Community Intervention Team

The Caredoc Community Intervention Team (CIT) is a nurse led professional team that provides acute nursing care to patients in the community setting. The service provides high quality nursing care for a short period of time by working together with patients, carers and other professionals. With the provision of this service suitable patients can return to their own home earlier from hospital or can be referred by their own General Practitioner to avoid a hospital attendance.

The service is currently available to patients in the counties of Carlow, Kilkenny, Waterford, Wexford, South Tipperary and South Wicklow and patients are referred to the service by hospital teams and General Practitioners conditional to certain clinical criteria.
